The patient was recently hospitalized for hematochezia and fever; blood and urine cultures at that time showed evidence of bacteremia, and she was therefore treated with a course of levofloxacin. Over the past 24 hours, the fever and chills have returned. Her past medical history includes aortobifemoral bypass graft surgery and coronary artery bypass graft surgery.
